Lead Nurse Practitioner
LOCATION:The Lead NP will supervise 8 nurse practitioners in 19 skilled nursing facilities in north Georgia. The SNFs are located in Athens, Atlanta, Baldwin, Blue Ridge, Brookhaven, Buford, Decatur, Elberton, Ft. Oglethorpe, Gainesville, Jasper, LaFayette, Lilburn, Rome, Toccoa, and Washington. The Lead NP can work out of any of these locations. Regular travel to these SNFs is required.
To develop our Nurse Practitioner (NP) program in support of PruittHealth Premier. This includes the establishment and implementation of protocols, policies, and procedures for NPs across their scope of work as well as the recruitment, selection, training, and leadership of these NPs.

5.AbilityTo effectively reconcilemedications.
6.Demonstratesability to identify and document the needs of clinically complexpatients.
7.Work collaboratively with physicians, casemanagers, social workers, family members, key care givers and any appropriateancillary medical personnel asappropriate.
8.Participates in and/or directs Quality AssurancePerformance Improvement (QAPI)activities.
9.Understands the PruittHealth Premier Planbenefits, PruittHealth Premier Plan policies, procedures and articulate themeffectively to providers, members and key decisionsmakers.
10.Assessesthe medical necessity/effectiveness of ancillary services to determine theappropriate initiation of benefit eventsand communicate the process to providers and appropriate team members.
11.Coordinatescare as member’s transition through different levels of care and care settings,ensures that the ICP is communicated Immediately and/or no later than 24 hoursto the receiving caresetting.
12.Establishesschedule according to approved PHP nurse practitionerworkflow
13.Establishesand documents visit schedule according the HRA stratification (low, moderate,high)
14.Ensuresall Members have an ICP developed within 48 hours ofHRAT
15.Facilitates agreement and implementation of themember's plan of care by engaging beneficiary, the facility staff,families/responsible parties, primary and specialty carephysicians
16.Evaluatesthe effectiveness, necessity and efficiency of the plan, making revisions asneeded
17.Utilizes practice guidelines and protocolsestablished by PruittHealth PremierPlanAdditionally, the Lead NP will serve in an active practicing NP role.
MINIMUM EDUCATION REQUIRED:
Graduate of an accredited Adult, Geriatric or Family NursePractitioner Master’s Degree program.
MINIMUM EXPERIENCE REQUIRED:
Minimum 5 years clinicalexperience as a Nurse Practitioner in geriatric, adult or family practicesetting.
Proven ability to performcomprehensive, adult/geriatric physical exam. Ability to lead and manager otherhealthcare professionals, particularly NPs.
Ability to use IT tools andprograms including EMR, Microsoft Office Suite, Excel Excellent communicationskills
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current and unrestricted, RN andNP licensure in state of practice. Board certification by AANP or ANCC, inAdult, Family Practice and Geriatrics or Adult-Gerontology Primary Care.Current CPR certification
ADDITIONAL QUALIFICATIONS: (Preferred qualifications)
Graduate of an accreditedGeriatric, Adult or Family Nurse Practitioner Master’s Degree program. Previouslong-term care experience strongly preferred.
KNOWLEDGE, SKILLS, ABILITIES:
Pertinent Clinical Work Experience which isrelevant to population beingserved.
Reviews and interprets the medical and socialhistory ofmembers
Interprets laboratory data, recommendappropriate follow-up, reconcilemedications
Manages both medical and behavioral chronic andacute conditions effectivelyincollaboration with a physician or specialtyprovider
Synthesizes information and easily diagnoses andcreates a comprehensive plan ofcare.
Ensures that individual plan of care isdeveloped with specific interventions designed to meet theneeds
identified in the Health RiskAssessment
Performs comprehensive assessments and documentfindings in aconcise/comprehensivemanner that is compliant with documentation requirements and Center forMedicare and Medicaid Services (CMS)regulations
Performs Health Risk Assessment within seven (7)days of enrollment for a newbeneficiaryand annually within one year of the initial assessment.
Conducts an annual Comprehensive Geriatric Examthat incorporatesCMSAnnual aWellness Visit elements, including the Personalized Prevention Plan Services(PPPS)
Ensures all diagnoses are ICD-10 codedaccurately and documented appropriately to support the diagnosis at thatvisit
Conducts HRA comprehensive face-to-face assessmentif the member experiences a change in condition and/or transition in care—a“trigger”event.
Reviews orders and interventions forappropriateness and response to treatment to identify most effective plan ofcare that aligns with the member's needs andwishes
Evaluates plan of care for cost effectivenesswhile meeting the needs of members,familiesand providers to decreases high costs, poor outcomes and unnecessaryhospitalizations
May be required to participate in on-callprogram, after hour or coverage may berequired
Rotating on callresponsibilities
Travels between care sites mandatory
Performs other duties asassigned
